Thank you for another very informative discussion. All three of you were great, but a special commendation to Dr. Foy for his crystal-clear summary of the FAME trial. I hadn't heard of the fragility index before and it sounds to me like a very useful measure to cite. The RIVAL trial took me back to my fellowship years in the 1970s when almost all of our access was through the femoral artery. Memories of standing there holding firm pressure for many minutes at the end of the procedure are still vivid. We did have one attending who used what was then called the Sones technique which involved a cutdown to the brachial artery for access and the use of the Sones catheters rather than what were then called Judkins catheters for femoral access. I wonder if the Sones technique is still used as an alternative if radial access cannot be used.
