As usual, a very valuable discussion. I was particularly glad to you endorse the value of digoxin which remained the mainstay in my treatment regimen for CHF and atrial fibrillation for 40 years. I tried a number of the newer "sexier" drugs but always came back with the old reliable digoxin. I did find the ACE inhibitors to be significantly helpful and incorporated them into standard CHF regimens. I didn't think the long acting nitrates were very helpful and the side effects from the earlier afterload reducers rendered them inferior to the ACE inhibitors.
